A Gentle First Step Toward Understanding
This page offers a collection of evidence‑based mental health screening questionnaires for children, adolescents, and adults. These tools are designed to help identify patterns, concerns, or areas that may benefit from additional support.
Completing a questionnaire does not provide a diagnosis. Instead, it helps me better understand your concerns so I can follow up with you, review results together, and determine whether scheduling an appointment would be helpful.
All responses are reviewed with care and confidentiality.
How This Works
Select the questionnaire that best fits your concerns or your child’s needs.
Complete the form honestly and thoughtfully.
I will review your responses.
I will reach out to discuss results and next steps, including the option to book an appointment.
If you are unsure which questionnaire to choose, feel free to reach out for guidance.
Featured Questionnaires
Ages: Children and Adolescents 6–12 (Parent/Caregiver Report)
The Vanderbilt Assessment Scale evaluates symptoms of Attention‑Deficit/Hyperactivity Disorder (ADHD). Parents or guardians complete an initial assessment to identify symptom frequency, academic functioning, and social performance.
A follow‑up assessment may be used to track changes in symptoms, functioning, and potential side effects over time.
Ages: Adolescents
The PHQ‑A is a screening tool used to assess symptoms of depression in adolescents. It helps identify emotional and behavioral patterns that may indicate a risk for depressive disorders.
Next Steps
After completing a questionnaire, I will review your responses and contact you to discuss results. Together, we can determine appropriate next steps, which may include scheduling an intake or follow‑up appointment.
If you have questions or need help choosing the right assessment, please don’t hesitate to reach out.
This page is intended for screening and informational purposes only and does not replace a comprehensive mental health evaluation.
