ADHD Assessment Scale – Parent Informant (VADRS)

Ages: Children and Adolescents 6–12 (Parent/Caregiver Report)

The Vanderbilt Assessment Scale evaluates symptoms of Attention‑Deficit/Hyperactivity Disorder (ADHD). Parents or guardians complete an initial assessment to identify symptom frequency, academic functioning, and social performance.

A follow‑up assessment may be used to track changes in symptoms, functioning, and potential side effects over time.

Adverse Childhood Experiences (ACE) Questionnaire

Ages: Adults

The ACE Questionnaire assesses exposure to childhood abuse, neglect, and household challenges. Research has shown that adverse childhood experiences can significantly impact long‑term physical health, emotional well‑being, relationships, and stress responses.

Understanding ACEs can provide valuable insight into current challenges and guide a more trauma‑informed approach to care.

Autism Spectrum Quotient‑10 for Children (AQ‑10 Child)

Ages: Children

The Autism Spectrum Quotient‑10 for Children is a short screening measure designed to assess traits commonly associated with autism in children. The 10 questions focus on social interaction, communication, and behavioral patterns.

Results can help identify whether further professional evaluation may be helpful.

The Columbia‑Suicide Severity Rating Scale is a clinician‑administered assessment used to gather detailed information about suicidal ideation and behaviors, both recent and across the lifespan.

While this tool does not provide a score or diagnosis, it helps identify suicide risk and supports clinical decision‑making regarding appropriate interventions and referrals.
